The following items must be initialed by the parent(s) stating that you agree and that you have complied with the policies before care begins:

• Signed Childcare Contract w/Registration Fee and Tuition

  Emergency Medical Form

  Photo Release

  Field Trip Permission

  Copy of Immunization Records

  Copy of Medical or Insurance Card

All information must be kept current. If there is any change, the parents do hereby agree that they shall notify the provider immediately.
A Registration Fee will also need to be received by the provider to ensure a position at Children's R Express.
There are NO REDUCTIONS in fees for holidays, sickness, inclement weather, vacation, or just taking the day off. YOU ARE RESPONSIBLE for payment of the days that you have agreed for your child to attend my care. There are no exceptions, and no overtime.
Credit cards are accepted online from our Rates Page.

Please note the Policy Handout will change from time to time, it is the parents' responsibility to check back frequently online or ask for an updated handbook for any noted changes. I will give up to a month notice of changes to be in effect.
